Community Health Center of Cape Cod

The Community Health Center of Cape Cod is your complete medical home. This means that our team of medical, mental health and dental providers can give you and your family all the services you need to achieve good health and well-being.

All of the activities that go into your care at the Health Center are coordinated by not just one medical provider but by a team of people who have different skills and training to work with you depending on your needs. Other members of our care teams include nursing staff, counselors, social workers, and registration staff. We are proud of the talents and dedication of all our staff.

Community Health Center of Cape Cod has three locations at:
• 123 Waterhouse Road, Bourne
• 210 Jones Road, Suite 22, Falmouth
• 107 Commercial Street, Mashpee. 

The Health Center is not a walk-in clinic. Because of the high demand for our services, patient appointments must be scheduled. Hours of service vary for each location and all appointments are made through a central scheduling number: 508 477-7090.
